CMS' new Medicare GLP-1 Bridge Demonstration Program is generating significant attention for expanding beneficiary access to weight management medications. While the program operates outside of the traditional Medicare Part D benefit, health plans should not assume they can take a hands-off approach.
Beginning July 1, 2026, the demonstration will provide eligible Medicare beneficiaries access to select GLP-1 medications through a centralized CMS-administered process. On the surface, the use of a central processor for eligibility, prior authorization, and claims payment may appear to minimize plan involvement. In reality, health plans will remain on the front lines of member and provider engagement.
Why Health Plans Should Pay Attention
Although Relay Health will serve as the central processor, beneficiaries and pharmacies will continue to look to their health plan when questions arise.
Health plans should anticipate increased volume and operational complexity in several key areas:
Member Inquiries
Members are unlikely to distinguish between a CMS demonstration program and their Medicare Part D benefit. Customer service teams should be prepared to answer questions regarding:
GLP-1 Bridge eligibility requirements
Covered medications and exclusions
Enrollment and access processes
Cost-sharing responsibilities
Differences between GLP-1 Bridge coverage and Part D coverage
Without adequate training, member confusion can quickly translate into dissatisfaction, complaints, and increased call center volume.
Pharmacy Support and Claims Troubleshooting
Participating pharmacies will need guidance when processing prescriptions and resolving claim issues.
Common questions may include:
Is the prescription eligible for GLP-1 Bridge or Part D coverage?
Which products are covered under the demonstration?
How should claims be routed?
What processing edits indicate eligibility or coverage issues?
Health plans should expect pharmacies to contact them when claims are rejected or when benefit determination is unclear.
Part D Appeals and Coverage Determinations
Even though the demonstration exists outside of Part D, plans must maintain oversight of:
Coverage determinations (CDs)
Part D appeals/ Redeterminations (RDs) for GLP-1 medications
Member complaints regarding coverage eligibility for GLP-1s
Clear internal workflows will be essential to ensure members receive accurate information and appropriate direction based on their clinical circumstances.
The Risk of a "Hands-Off" Approach
One of the biggest misconceptions surrounding the GLP-1 Bridge Demonstration is that plans can simply direct all questions back to CMS.
While CMS has centralized program administration, plans remain a trusted point of contact for members, providers, and pharmacies. Organizations that fail to prepare operationally may experience:
Increased call center burden
Escalated grievances and complaints
Provider and pharmacy frustration
Member confusion regarding coverage pathways
Compliance and oversight challenges
How Health Plans Can Prepare
Successful implementation will require cross-functional coordination across:
Pharmacy Operations
Medicare Compliance
Customer Service
Appeals & Grievances
Utilization Management
Clinical Operations
Plans should begin evaluating staff training needs, member communication strategies, pharmacy support processes, and escalation pathways before the program launch.
The GLP-1 Bridge Demonstration may be administered outside of Part D, but the operational impact will be felt across the organization.
